Write the purpose of the visit, the questions you want answered and any documents the provider requested. Check the location and arrival instructions. Keep sensitive details in a place appropriate for them, and use paper if you prefer not to open a device during the conversation.
Leave room for the answer
Under each question, leave space to record the response. At the end, confirm any next step you do not understand. Ask where to find written instructions if the details are too much to capture accurately in your own notes.
Afterwards, put follow-up dates into the calendar and file the note with the relevant records. You do not need a complicated appointment system. For the next visit, a copy of the same simple structure may be enough, with questions changed to suit what you actually need to learn.
